Who we are

ClearToCare is a service of The Advanced Practice Network LLC (“we,” “us”). We provide digital-first mandatory employee health screenings, including OSHA respirator medical evaluations reviewed by licensed clinicians. Questions about this policy can be sent to support@cleartocare.com.

Information we collect

Respirator evaluation responses. When you complete the respirator medical evaluation, we collect the identifying information and health information you provide (such as your name, contact details, and answers to OSHA's required medical questions). The questionnaire is collected through Google Forms under our organization's Google Workspace account.

Contact and partner inquiries. When you contact us or submit the partner form, we collect the information you provide, such as your name, email, phone number, company, and message.

How we use your information

We use your information to perform your medical evaluation, issue your clearance determination, contact you if a follow-up is needed, respond to inquiries, and operate and improve our services. We do not sell your personal information.

Who sees your health information

Your medical questionnaire responses are reviewed only by the licensed clinician performing your evaluation. Your employer receives only your clearance determination — never your medical answers.

Service providers

We use a small number of service providers to operate this website and our evaluation process, such as website hosting, form collection, and email delivery. These providers process information on our behalf and are not permitted to use it for their own purposes.

Security & retention

Information is transmitted over encrypted connections and access is limited to those who need it to provide our services. We retain evaluation records as required by applicable occupational health recordkeeping requirements, and other information only as long as needed for the purposes described above.

Your choices

To ask about, correct, or request deletion of information we hold about you, email support@cleartocare.com or call/text (270) 391-0697. Note that some evaluation records must be retained to meet legal and regulatory requirements.

Changes to this policy

If we make material changes to this policy, we will update this page and its “Last updated” date.
